Nearly 1 million people in England have now joined the NHS Healthier You programme to prevent or delay Type 2 Diabetes. We are a proud provider of the NDPP across 15 regions, providing structured, educational support.

How Can Community Organisations Support Us?

Our aim is to ensure people of of all ages and backgrounds who are at risk of developing type 2 diabetes are aware of, and able to access, this free NHS programme.

We are keen to connect with community organisations like yours to help us reach more individuals who would benefit from this 9-month programme designed to support healthier lifestyles and long-term wellbeing.

If you run any community events, groups, meetings or outreach activities where we could share information about the NDPP with members of the public, we would love to hear from you!

What is the NHS Diabetes Prevention Programme?

The Healthier You: NHS Diabetes Prevention Programme (NDPP) is a joint initiative between NHS England & Improvement (NHSE&I), Office for Health Improvement & Disparities (OHID)

[formerly Public Health England] and Diabetes UK.

It delivers services across the whole of England for those with prediabetes or women with a previous history of gestational diabetes, who are at high risk of developing Type 2 diabetes.

 

Expected Outcomes of the NDPP:

Reduce the incidence of Type 2 diabetes as a result of the intervention.

Reduction in weight of Service Users where they are overweight or obese, and the maintenance of a healthy weight; and

Reduce blood glucose parameters (HbA1c or Fasting Plasma Glucose (FPG)) in Service Users at 12 months from referral and beyond.

Who is Eligible for the NDPP?

 

How to Access the NDPP?

Eligible individuals can either self-register or be directly referred to the NDPP by their GP.

For those with prediabetes (non-diabetes hyperglycaemia), self-registration is available via: https://preventing-diabetes.co.uk/referral/

 For women with a current or previous diagnosis of gestational diabetes (GDM), self-registration is available via:

Please note: Women who are currently pregnant will be placed on hold and recontacted 1-month post-birth to begin the programme.
