Digital Comfort Tracker App Volunteer with Palliative and End of Life Care Service

Sussex Community NHS Foundation Trust are looking for volunteers to support with the roll out of a innovative new app which enables patients and carers to log symptoms from home which are reviewed by health care teams on a central dashboard. Volunteers will be based at SCFT premises and will be contacting patients to:

Explain what the app is and how it can help
Gain the patient/carer's consent to use the app
Obtain required patient information for healthcare professionals
